Basic requirements

  • 2 valid IDs (applicant and spouse or co-maker)
  • Billing statement at your present address
  • Mode of payment: auto-debit through BDO or BPI

For employed applicants

  • Certificate of Employment (COE)
  • 3 months' payslips or latest ITR 2316
  • 3 months' bank statements or payroll records

OFW / Seaman

  • Certificate of Employment (COE) or employment contract
  • Proof of remittance
  • 3 months' bank statements or payroll records

Self-employed / with business

  • Mayor's Permit
  • DTI Registration or SEC Registration
  • ITR 2307 or 1701
  • 4–6 months' bank statements

Corporation

  • SEC Registration
  • Articles of Incorporation
  • Latest ITR and AFS
  • Last 4 months' bank statements
  • Secretary's Certificate
